Sample item

A boy has unilateral renal agenesis on ultrasound and is later found to lack the ipsilateral vas deferens. What explains this association?

Original practice item, not a recalled exam question. Nothing here is recorded.

The answer and the reasoning

C. Defective mesonephric duct, which gives rise to both the ureteric bud and the vas deferens

The ureteric bud arises from the mesonephric duct and induces the metanephric blastema to form the kidney. The mesonephric duct also gives rise to the vas deferens, epididymis, seminal vesicle and ejaculatory duct in males. Failure of the duct therefore causes renal agenesis together with absence of the vas deferens.

Option by option

  • A. Defective paramesonephric duct, which gives rise to both the ureteric bud and the vas deferens

The paramesonephric duct regresses in males and does not form the ureteric bud.

  • B. Defective metanephric blastema, which gives rise to both the nephrons and the vas deferens

The blastema forms nephrons but not the vas deferens, which is a duct derivative.

  • C. Defective mesonephric duct, which gives rise to both the ureteric bud and the vas deferens (correct)

Correct. The mesonephric duct gives rise to both the ureteric bud and the vas deferens.

  • D. Defective urogenital sinus, which gives rise to both the ureter and the vas deferens

The urogenital sinus forms the bladder and urethra, not the vas deferens or ureteric bud.

Questions about this Anatomy question

What is the answer to this Anatomy question?

The best answer is C, Defective mesonephric duct, which gives rise to both the ureteric bud and the vas deferens. The ureteric bud arises from the mesonephric duct and induces the metanephric blastema to form the kidney.

Why is "Defective paramesonephric duct, which gives rise to both the ureteric…" not the answer?

The paramesonephric duct regresses in males and does not form the ureteric bud.

Why is "Defective metanephric blastema, which gives rise to both the nephrons…" not the answer?

The blastema forms nephrons but not the vas deferens, which is a duct derivative.

Why is "Defective urogenital sinus, which gives rise to both the ureter and t…" not the answer?

The urogenital sinus forms the bladder and urethra, not the vas deferens or ureteric bud.

What concept does this question test?

It tests Embryology: urinary system in Anatomy. The mesonephric duct also gives rise to the vas deferens, epididymis, seminal vesicle and ejaculatory duct in males. Failure of the duct therefore causes renal agenesis together with absence of the vas deferens.

How hard is this question?

We tag it as a multi-step question: it asks you to combine more than one idea. The tag is our own editorial judgement, not an official difficulty rating, so read the reason for every option, not only the answer.
