PSM · Immunisation: vaccine vial monitor
Sample itemA health worker checks the vaccine vial monitor (VVM) on a vial of measles-rubella vaccine. The inner square is darker than the outer circle. What should be done?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
D. Discard the vial, because it has been exposed to too much heat
The vaccine vial monitor is a heat-sensitive label that darkens with cumulative heat exposure. If the inner square is the same colour as, or darker than, the outer circle, the vaccine has reached its discard point and must not be used. If the inner square is lighter than the circle, the vial can be used, with the earliest expiry used first. VVM records heat exposure but does not detect freezing, for which the shake test is used.
Option by option
A. Keep it for the next session in the freezer
A freezer would not help, because heat damage cannot be reversed and measles-rubella vaccine is stored at +2 to +8 °C.
B. Use it only for adults
Heat-damaged vaccine loses potency in all age groups.
C. Use the vial first, because it is near expiry
A VVM reading reflects heat damage, not expiry date, and an over-exposed vial is unsafe to use.
D. Discard the vial, because it has been exposed to too much heat (correct)
Correct. A darker inner square means the discard point has been passed.
