Biochemistry · Deoxyribonucleotide synthesis: hydroxyurea

Sample item

A 9-year-old with sickle cell disease and recurrent painful crises is started on hydroxyurea. Which enzymatic step in nucleotide metabolism does it directly inhibit?

Original practice item, not a recalled exam question. Nothing here is recorded.

The answer and the reasoning

B. Ribonucleotide reductase, which converts ribonucleoside diphosphates to deoxyribonucleoside diphosphates

Hydroxyurea quenches the tyrosyl free radical required by the R2 subunit of ribonucleotide reductase, lowering deoxyribonucleotide pools and slowing DNA synthesis in rapidly dividing cells. In sickle cell disease it raises fetal haemoglobin, reducing sickling, and it also reduces neutrophil counts. Myelosuppression is its main toxicity.

Option by option

  • A. Dihydrofolate reductase

Dihydrofolate reductase is inhibited by methotrexate and trimethoprim, not hydroxyurea.

  • B. Ribonucleotide reductase, which converts ribonucleoside diphosphates to deoxyribonucleoside diphosphates (correct)

Ribonucleotide reductase inhibition limits dNTP supply and is the main target of hydroxyurea.

  • C. Thymidylate synthase, which converts dUMP to dTMP

Thymidylate synthase is inhibited by the metabolite of fluorouracil.

  • D. IMP dehydrogenase

IMP dehydrogenase is the target of mycophenolate in guanine nucleotide synthesis.

Questions about this Biochemistry question

What is the answer to this Biochemistry question?

The best answer is B, Ribonucleotide reductase, which converts ribonucleoside diphosphates to deoxyribonucleoside diphosphates. Hydroxyurea quenches the tyrosyl free radical required by the R2 subunit of ribonucleotide reductase, lowering deoxyribonucleotide pools and slowing DNA synthesis in rapidly dividing cells.

Why is "Dihydrofolate reductase" not the answer?

Dihydrofolate reductase is inhibited by methotrexate and trimethoprim, not hydroxyurea.

Why is "Thymidylate synthase, which converts dUMP to dTMP" not the answer?

Thymidylate synthase is inhibited by the metabolite of fluorouracil.

Why is "IMP dehydrogenase" not the answer?

IMP dehydrogenase is the target of mycophenolate in guanine nucleotide synthesis.

What concept does this question test?

It tests Deoxyribonucleotide synthesis: hydroxyurea in Biochemistry. In sickle cell disease it raises fetal haemoglobin, reducing sickling, and it also reduces neutrophil counts. Myelosuppression is its main toxicity.

How hard is this question?

We tag it as an application question: it asks you to use a fact on a patient or a finding. The tag is our own editorial judgement, not an official difficulty rating, so read the reason for every option, not only the answer.

How should I study Deoxyribonucleotide synthesis: hydroxyurea?

Build one table of enzyme deficiency, disease, inheritance and key finding, then revise it by spaced recall. Then use the flashcards and the other Biochemistry questions on this site to test what stuck.

Is this a real NEET PG question?

No. It is an original practice item (editorial status: sample, version 1). NextPrep does not publish recalled or leaked questions, and independent faculty review of this item has not been completed yet, so confirm important facts in a standard textbook.
