General Surgery · Hernia and abdominal wall: pantaloon hernia

Sample item

At operation for a groin hernia, two separate sacs are found, one lateral and one medial to the inferior epigastric vessels, straddling the vessels. What is this called?

Original practice item, not a recalled exam question. Nothing here is recorded.

The answer and the reasoning

C. Pantaloon hernia

A pantaloon (saddlebag) hernia has both a direct and an indirect sac, on either side of the inferior epigastric vessels. Both must be dealt with at repair, and a mesh repair that covers the whole myopectineal orifice does this.

Option by option

  • A. Maydl hernia

A Maydl hernia has two loops of bowel within the sac with the connecting loop in the abdomen.

  • B. Spigelian hernia

A Spigelian hernia occurs through the linea semilunaris, lateral to the rectus sheath.

  • C. Pantaloon hernia (correct)

Correct. Direct and indirect sacs on either side of the inferior epigastric vessels form a pantaloon hernia.

  • D. Sliding hernia

A sliding hernia has a retroperitoneal organ forming part of the sac wall.
