General Surgery · Vascular surgery: DVT treatment
Sample itemA 40-year-old has a confirmed popliteal vein DVT after a long flight and no cancer. What is the standard treatment?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
A. Anticoagulation for at least three months
Therapeutic anticoagulation with LMWH or a direct oral anticoagulant, with or without a warfarin bridge, prevents propagation and embolism, and a provoked DVT is treated for at least three months. IVC filters are for those with contraindication to anticoagulation or recurrent embolism despite it. Early mobilisation with compression is encouraged.
Option by option
A. Anticoagulation for at least three months (correct)
Correct. At least three months of anticoagulation is standard.
B. Bed rest alone
Rest alone does not prevent propagation.
C. Aspirin for one week
Aspirin is inadequate.
D. Immediate IVC filter in all cases
Filters are reserved for special circumstances.
