General Surgery · Vascular surgery: duplex scan in varicose veins
Sample itemA woman with symptomatic varicose veins and aching legs is considering endovenous ablation. Which investigation is essential before intervention?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
D. Colour duplex ultrasound mapping of reflux
Duplex ultrasound shows reflux at the saphenofemoral or saphenopopliteal junction, the saphenous trunks and perforators, and the state of the deep veins, and so directs treatment.
Option by option
A. Contrast venography as the routine test
Venography is invasive and not routine.
B. ABI only
ABI assesses arterial supply and does not map reflux.
C. CT angiography of the legs
CT angiography images arteries, not venous reflux.
D. Colour duplex ultrasound mapping of reflux (correct)
Correct. Duplex scanning is the standard test before treatment.
