Sample itemA woman with postpartum haemorrhage has lost 3 L of blood, and the fibrinogen is 0.8 g/L despite red cells and plasma. Which product is the most concentrated source of fibrinogen?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
C. Cryoprecipitate
Cryoprecipitate is rich in fibrinogen, factor VIII, von Willebrand factor and factor XIII, and a small volume raises the fibrinogen substantially. Fresh frozen plasma contains fibrinogen at a much lower concentration, so large volumes would be needed.
Option by option
A. Fresh frozen plasma
Plasma contains fibrinogen only at physiological concentration and would need a large volume.
B. Platelet concentrate
Platelets do not supply a meaningful amount of fibrinogen.
C. Cryoprecipitate (correct)
Correct. Cryoprecipitate concentrates fibrinogen in a small volume.
D. Packed red cells
Red cells contain no clotting factors.
