Sample itemWhat is the first investigation of choice in suspected acute subarachnoid haemorrhage?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
C. Non-contrast CT of the head
Non-contrast CT is highly sensitive for blood in the first hours. If CT is negative but suspicion remains, lumbar puncture looking for xanthochromia follows. CT or catheter angiography then locates the aneurysm.
Option by option
A. Cerebral angiography
Angiography is used after the bleed is confirmed.
B. MRI of the brain
MRI is not the first-line test in an emergency.
C. Non-contrast CT of the head (correct)
Correct.
D. Lumbar puncture
Lumbar puncture is done after a negative CT.
