Pharmacology · Cardiovascular: mannitol contraindication
Sample itemIn which situation is mannitol contraindicated?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
B. Active intracranial haemorrhage with a bleeding source not controlled
Mannitol is avoided in active intracranial bleeding because osmotic shrinkage can enlarge the haematoma, and it is also avoided in severe heart failure and pulmonary oedema because of volume expansion. It remains a standard agent for cerebral oedema and acute glaucoma.
Option by option
A. Acute glaucoma awaiting surgery
Acute glaucoma is an indication.
B. Active intracranial haemorrhage with a bleeding source not controlled (correct)
Correct. It can worsen bleeding when the vessel is not controlled.
C. Raised intracranial pressure from cerebral oedema
Raised intracranial pressure is an indication.
D. Oliguria with preserved renal function
Preserved renal function is not a contraindication.
