ENT · Ear and audiology: electroneuronography
Sample itemTen days after onset of complete facial palsy from a temporal bone injury, electroneuronography shows more than 90 percent degeneration. What does this imply?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
B. Poor prognosis, so surgical decompression is considered
Electroneuronography compares the evoked compound action potential on the two sides and estimates the percentage of degenerated fibres. Degeneration exceeding 90 percent within 3 weeks predicts incomplete recovery and supports decompression. The test is useful only after about 3 days when Wallerian degeneration is established.
Option by option
A. Excellent prognosis with spontaneous recovery
Recovery is likely when degeneration is below 90 percent.
B. Poor prognosis, so surgical decompression is considered (correct)
Correct. Rapid severe degeneration predicts a poor result.
C. Need only for eye care
Eye care is needed, but the test directs timing of surgery.
D. That the lesion is central
A central lesion would spare the forehead.
