ENT · Larynx and airway: posterior glottic stenosis
Sample itemAfter 3 weeks of mechanical ventilation, a patient has stridor on exertion and limited cord abduction with a fibrous band between the arytenoids. What is the diagnosis?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
B. Posterior glottic stenosis from prolonged intubation
Prolonged intubation causes pressure ulceration in the posterior glottis, with interarytenoid scarring and fixation of the arytenoids. Distinguishing it from bilateral cord paralysis needs palpation of the arytenoids at microlaryngoscopy. Treatment is scar division, laser or posterior cordotomy.
Option by option
A. Laryngeal web from birth

B. Posterior glottic stenosis from prolonged intubation (correct)
Correct. Posterior scarring follows prolonged intubation.
C. Bilateral recurrent laryngeal palsy
Palsy shows mobile arytenoids on palpation.
D. Laryngeal carcinoma
Cancer shows a mass lesion.
