OBG · Normal pregnancy: circumvallate placenta
Sample itemA placenta examined after delivery has a thick, raised, greyish-white ring on the fetal surface with the membranes folded back on themselves. In which complication is this variant linked?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
D. Antepartum haemorrhage and preterm delivery
In circumvallate placenta the chorionic plate is smaller than the basal plate, and the membranes double back on themselves to form a thick ring. It is associated with antepartum bleeding, preterm labour and growth restriction. It is usually discovered after delivery.
Option by option
A. Vasa praevia
Vasa praevia is related to velamentous cord insertion.
B. Placenta accreta
Accreta is associated with praevia and scars, not circumvallate shape.
C. Fetal polycythaemia
Polycythaemia is associated with twin transfusion or maternal diabetes.
D. Antepartum haemorrhage and preterm delivery (correct)
Correct. The variant is linked with antepartum bleeding and preterm birth.
