OBG · Endometriosis: ultrasound of endometrioma
Sample itemA 30-year-old with dysmenorrhoea and infertility has a 5 cm unilocular ovarian cyst with homogeneous low-level internal echoes (ground-glass appearance) and no solid papillary projections or flow. What is the most likely diagnosis?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
A. Endometrioma
An endometrioma, or chocolate cyst, classically appears as a unilocular cyst with ground-glass echoes from old blood, often with other signs such as kissing ovaries. Dermoids show echogenic nodules and acoustic shadows, and malignant tumours have solid components with vascularity. A simple cyst is anechoic.
Option by option
A. Endometrioma (correct)
Correct. Homogeneous low-level echoes in a unilocular cyst typify an endometrioma.
B. Serous cystadenocarcinoma
A malignant tumour would show solid papillary areas with flow.
C. Dermoid cyst
A dermoid shows echogenic areas and shadowing, not uniform ground glass.
D. Simple follicular cyst
A simple cyst is clear and anechoic.
