OBG · Contraception: post-abortion contraception
Sample itemA woman has an uncomplicated first-trimester surgical abortion and wants long-term contraception. What is the advice regarding IUCD insertion?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
A. It can be inserted immediately after the procedure if there is no infection
Fertility returns within about 2 weeks after abortion, so contraception should start immediately. IUCDs or the levonorgestrel IUS can be inserted straight after an uncomplicated first-trimester abortion, with a low expulsion rate, but not in the presence of sepsis. Hormonal methods can also be started the same day.
Option by option

B. It must be delayed for 3 months
Delay is not needed in uncomplicated cases.
C. It is contraindicated after any abortion
IUCDs are not contraindicated after clean abortion.
