Sample item

A 40-year-old woman has a 4.5 cm squamous cervical carcinoma confined to the cervix, without parametrial extension or nodal spread on imaging. What is the FIGO 2018 stage and standard treatment?

Original practice item, not a recalled exam question. Nothing here is recorded.

The answer and the reasoning

C. Stage IB3; concurrent chemoradiation with cisplatin and brachytherapy

FIGO 2018 divides stage IB by size of the invasive tumour: IB1 under 2 cm, IB2 2 to 4 cm and IB3 4 cm or more. A tumour of 4 cm or more confined to the cervix is stage IB3, for which the standard of care is concurrent cisplatin-based chemoradiation followed by brachytherapy. Stages up to IB2 are usually treated surgically.

Option by option

  • A. Stage IVA; palliation only

Stage IVA means invasion of bladder or rectal mucosa.

  • B. Stage IIB; simple hysterectomy

    Stage IIB requires parametrial invasion.

  • C. Stage IB3; concurrent chemoradiation with cisplatin and brachytherapy (correct)

Correct. A 4.5 cm confined tumour is IB3 and is treated with chemoradiation.

  • D. Stage IA2; cone biopsy

Stage IA2 is microscopic invasion of 3 to 5 mm.
