ENT · Ear and audiology: glycerol test in Meniere disease
Sample itemIn suspected early Meniere disease, which finding after oral glycerol dehydration supports hydrops?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
B. Improvement of the pure tone threshold by at least 10 dB in the affected ear
The glycerol test (Klockhoff) dehydrates the endolymph through osmotic action. In hydrops a temporary improvement in the threshold, typically 10 dB or more, or in speech discrimination, is considered positive. The test is not used much now because of side effects and low sensitivity.
Option by option
A. No change in thresholds and speech discrimination
No change is a negative result.
B. Improvement of the pure tone threshold by at least 10 dB in the affected ear (correct)
Correct. Temporary threshold improvement follows reduction of endolymphatic pressure.
C. Appearance of nystagmus towards the normal ear
Nystagmus is not a response measured in this test.
D. A 20 dB fall in hearing
A fall in hearing is not the response to dehydration.
