OBG · Benign gynaecology: McCall culdoplasty
Sample itemDuring vaginal hysterectomy for uterovaginal prolapse, the surgeon approximates the uterosacral ligaments across the midline and closes the peritoneum high up. What is the purpose of this step?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
C. Prevention of enterocele and vault prolapse
McCall culdoplasty obliterates the pouch of Douglas and supports the vaginal vault by plicating the uterosacral ligaments. This reduces the risk of later enterocele and vault prolapse. It does not control bleeding or treat incontinence.
Option by option
A. Control of uterine artery bleeding
Uterine artery bleeding is controlled by clamping and ligating the pedicles.
B. Prevention of vesicovaginal fistula
Fistula prevention depends on careful bladder dissection, not this step.
C. Prevention of enterocele and vault prolapse (correct)
Correct. It supports the vault and obliterates the cul-de-sac.
D. Closure of the bladder neck for continence
Bladder neck support needs a sling or colposuspension.
