OBG · Benign gynaecology: route of hysterectomy
Sample itemA 50-year-old woman needs hysterectomy for a 10-week-size fibroid uterus, with no prolapse or previous pelvic surgery. Which approach is generally preferred when feasible?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
C. Vaginal hysterectomy
Vaginal hysterectomy is generally preferred when feasible because of shorter recovery, fewer wound problems and lower cost. Laparoscopic assistance is chosen if vaginal access is limited, and abdominal hysterectomy is reserved for large or complex uteri. Radical surgery is only for cancer.
Option by option
A. Subtotal abdominal hysterectomy
Subtotal surgery leaves the cervix and is not routine.
B. Radical hysterectomy
Radical hysterectomy is for cervical cancer, not a benign uterus.
C. Vaginal hysterectomy (correct)
Correct. Vaginal route is preferred when it is feasible.
D. Open abdominal hysterectomy
An open approach carries more morbidity.
