Psychiatry · Forensic psychiatry: capacity
Sample itemA patient can state the proposed operation but cannot explain in their own words the material risks, benefits, or alternatives, even after support is provided. Which decisional ability is primarily impaired?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
D. Understanding relevant information
Capacity is decision-specific and generally includes the abilities to understand relevant information, retain it long enough to decide, use or weigh it, and communicate a choice. A diagnosis alone does not establish incapacity.
Option by option
A. Communicating a choice
The patient may be able to communicate words while still not understanding their meaning.
B. Having a psychiatric diagnosis
Mental illness does not by itself remove decision-making capacity.
C. Agreeing with the clinician
Capacity does not require making the choice preferred by the clinician.
D. Understanding relevant information (correct)
Correct. Inability to grasp the decision-relevant information indicates impaired understanding.
