Sample itemIn which condition is the uterotonic methylergometrine contraindicated for management of postpartum haemorrhage?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
D. Hypertension or pre-eclampsia
Methylergometrine causes sustained uterine contraction through serotonergic and alpha-adrenergic receptors, and it also produces vasoconstriction. This can precipitate dangerous hypertension, so it is avoided in hypertension, pre-eclampsia, and cardiac disease. Carboprost is avoided in asthma.
Option by option
A. Retained products after incomplete abortion
It may be used to contract the uterus after evacuation if there is bleeding.
B. Prolonged labour
Prolonged labour is a risk factor for atony, which it treats, so it is not a contraindication.
C. Uterine atony
Uterine atony is the main indication for uterotonics.
D. Hypertension or pre-eclampsia (correct)
Correct. Vasoconstriction and hypertensive crisis are the main hazards.
