OBG · Labour and delivery: amnioinfusion
Sample itemA woman in labour with oligohydramnios develops repetitive variable decelerations with normal baseline rate and variability. Which intervention may relieve cord compression?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
A. Transcervical amnioinfusion with warmed isotonic saline
Variable decelerations in oligohydramnios are due to cord compression. Replacing fluid with warmed normal saline through an intrauterine catheter cushions the cord and often relieves the decelerations. Maternal position change and stopping oxytocin are first steps.
Option by option
A. Transcervical amnioinfusion with warmed isotonic saline (correct)
Correct. Replacing liquor cushions the cord and reduces compression.
B. Fundal pressure during contractions
Fundal pressure increases uterine pressure on the cord and is harmful.
C. Restriction of oral and intravenous fluids
Restricting fluids would not increase the liquor volume around the cord.
D. Intravenous magnesium sulphate
Magnesium sulphate has no role in relieving cord compression.
