OBG · Gynaecological oncology: uterine artery embolisation
Sample itemA 40-year-old with symptomatic 6 cm fibroids has completed her family and wishes to avoid hysterectomy. Which minimally invasive treatment obliterates the blood supply of the fibroids?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
D. Uterine artery embolisation
Uterine artery embolisation is done through the femoral artery and occludes the uterine arteries with particles, which shrinks the fibroids. It is a uterus-sparing option for women who do not want fertility. It is not recommended when a woman wishes to conceive.
Option by option
A. Myolysis by hysteroscopy
Hysteroscopic procedures treat submucous fibroids only.
B. Endometrial ablation
Endometrial ablation treats the lining only and does not treat large intramural fibroids.
C. High-dose oral progestins
Progestins control bleeding but do not shrink fibroids reliably.
D. Uterine artery embolisation (correct)
Correct. Embolising the uterine arteries starves the fibroids.
