Pharmacology · Chemotherapy: belimumab
Sample itemA woman with active systemic lupus erythematosus, positive for anti-dsDNA, remains symptomatic on hydroxychloroquine and prednisolone. Belimumab is added. Which target does it neutralise?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
D. B-lymphocyte stimulator (BLyS or BAFF)
Belimumab is a monoclonal antibody that binds soluble BLyS (BAFF), a survival factor for B cells, reducing autoreactive B-cell survival. It is approved for SLE and lupus nephritis. Rituximab depletes CD20-positive B cells, and eculizumab blocks C5.
Option by option
A. Complement component C5
C5 is the target of eculizumab.
B. Interleukin-17A
IL-17A is the target of secukinumab.
C. CD20 on B cells
CD20 is the target of rituximab.
D. B-lymphocyte stimulator (BLyS or BAFF) (correct)
Correct. Belimumab neutralises BLyS (BAFF).
