OBG · Gynaecological endocrinology: Y chromosome material in Turner phenotype
Sample itemA 16-year-old with Turner phenotype has primary amenorrhoea, streak gonads and a 45,X/46,XY karyotype. What is advised?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
B. Prophylactic bilateral gonadectomy
Gonads containing Y chromosome material carry a substantial risk of gonadoblastoma and dysgerminoma. Streak gonads are removed once the diagnosis is made, and oestrogen replacement is then given for puberty and bone health.
Option by option
A. Annual ultrasound surveillance only
Surveillance cannot reliably detect a gonadoblastoma at a curable stage.
B. Prophylactic bilateral gonadectomy (correct)
Correct. Y-bearing streak gonads are removed because of gonadoblastoma risk.
C. Unilateral oophorectomy
Removing one gonad leaves the other at risk.
D. Oestrogen therapy without surgery
Oestrogen alone leaves the malignancy risk untreated.
