Sample item

In which face presentation can a vaginal delivery occur?

Original practice item, not a recalled exam question. Nothing here is recorded.

The answer and the reasoning

C. Mentoanterior

A mentoanterior face allows the chin to pivot under the symphysis and the face to deliver by flexion. A persistent mentoposterior position cannot deliver vaginally because the neck cannot extend further, and needs caesarean section. Face presentation is diagnosed when the examining finger feels mouth, nose and malar eminences.

Option by option

  • A. Mentoposterior with persistent chin position

Persistent mentoposterior position leads to obstruction.

  • B. None, all require caesarean section

Vaginal delivery is possible in the mentoanterior position.

  • C. Mentoanterior (correct)

Correct. Mentoanterior face can deliver by flexion.

  • D. Mentotransverse that never rotates

A mentotransverse position that fails to rotate becomes arrested.
