Radiology · Breast imaging: BI-RADS
Sample itemOn screening mammography, a radiologist assigns BI-RADS category 4. What is the recommended management?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
D. Tissue diagnosis (biopsy)
BI-RADS 4 means suspicious abnormality with a 2 to 94 percent likelihood of malignancy, requiring biopsy. BI-RADS 3 (probably benign) is followed at short intervals, 0 needs additional imaging, 5 is highly suggestive of malignancy, and 6 is known malignancy. Category 1 and 2 return to routine screening.
Option by option
A. Short-interval follow-up at 6 months
Six-month follow-up applies to category 3.
B. Immediate definitive treatment without a biopsy
Treatment needs histological confirmation.
C. Routine annual screening
Routine screening applies to categories 1 and 2.
D. Tissue diagnosis (biopsy) (correct)
Correct. Category 4 needs a biopsy.
