Ophthalmology · Lens and cataract: toric intraocular lens
Sample itemA cataract patient has 2.5 dioptres of regular corneal astigmatism and wants spectacle independence for distance. Which implant best suits?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
D. Toric monofocal intraocular lens
A toric IOL, aligned with the steep corneal axis, corrects regular corneal astigmatism at the time of cataract surgery. Spherical, multifocal and accommodating lenses address power and presbyopia but do not correct meaningful astigmatism. Astigmatism must be regular and stable.
Option by option
A. Accommodating intraocular lens
An accommodating lens aims at near vision and does not correct astigmatism.
B. Standard spherical monofocal intraocular lens
A spherical lens leaves the 2.5 dioptres of astigmatism uncorrected.
C. Multifocal intraocular lens
A multifocal lens targets presbyopia and does not correct this degree of astigmatism.
D. Toric monofocal intraocular lens (correct)
Correct. A toric lens neutralises regular corneal astigmatism.
