Pediatrics · Paediatric infections: urinary tract infection imaging
Sample itemA 5-month-old girl has a first febrile urinary tract infection that responds to treatment. Which investigation should be done?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
D. Renal and bladder ultrasound
All infants with a first UTI need ultrasound to find obstruction or structural abnormality. A micturating cystourethrogram is added if the ultrasound is abnormal or infection recurs. DMSA is used to look for scarring later.
Option by option
A. CT of the abdomen
CT gives high radiation and is not indicated.
B. Micturating cystourethrogram in every case
Routine cystourethrogram is reserved for abnormal ultrasound or recurrence.
C. Intravenous urography
Urography exposes the child to contrast and radiation, and is outdated.
D. Renal and bladder ultrasound (correct)
Correct. Ultrasound is the first imaging study.
