OBG · Benign gynaecology: assessment of stress incontinence
Sample itemA multiparous woman reports leakage of urine on coughing. Which bedside test assesses urethral hypermobility?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
A. Q-tip (cotton swab) test, with a deflection of more than 30 degrees on straining
In stress urinary incontinence from urethral hypermobility the cotton swab placed in the urethra rotates by more than 30 degrees from horizontal on straining. A cough stress test confirms leakage, and urodynamic studies are done before surgery in complex cases. The Bonney test is an older manoeuvre of elevating the bladder neck.
Option by option
A. Q-tip (cotton swab) test, with a deflection of more than 30 degrees on straining (correct)
Correct. A swab deflection above 30 degrees indicates urethral hypermobility.
B. Schiller iodine test
The Schiller test is used for cervical lesions.
C. Whiff test
The whiff test is for bacterial vaginosis.
D. Bonney test only
The Bonney test is not the standard test of mobility and is less used today.
