OBG · Gynaecological oncology: screening after hysterectomy

Sample item

A 55-year-old woman has had a total hysterectomy for fibroids with a normal cervix, and no previous history of CIN 2 or worse. What is the recommendation for cervical screening?

Original practice item, not a recalled exam question. Nothing here is recorded.

The answer and the reasoning

A. No further screening is needed

After total hysterectomy with removal of the cervix for benign disease, in women with no history of high-grade CIN or cervical cancer, further cervical screening is not necessary. Women with a prior high-grade lesion continue to have vaginal vault cytology. Women with a retained cervix after subtotal hysterectomy continue routine screening.

Option by option

  • A. No further screening is needed (correct)

Correct. Screening stops after total hysterectomy for benign disease without prior CIN 2 or worse.

  • B. Screening every 3 years until 80

Screening intervals do not apply when there is no cervix.

  • C. Annual colposcopy

Colposcopy is not used for routine screening.

  • D. Annual cytology for life

Annual cytology is not indicated for a woman without a cervix.
