OBG · Gynaecological oncology: myometrial invasion in endometrial cancer
Sample itemA woman has an endometrioid endometrial carcinoma that invades less than half of the myometrium and has no cervical or adnexal involvement. What is its FIGO 2009 stage?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
B. Stage IA
In FIGO staging, stage IA is confined to the uterus with no or less than 50 percent myometrial invasion, and stage IB has invasion of 50 percent or more. Stage II involves the cervical stroma, and stage III involves serosa, adnexa, vagina or nodes. Pelvic MRI helps evaluate depth of invasion preoperatively.
Option by option
A. Stage IB
Stage IB requires invasion of half or more of the myometrium.
B. Stage IA (correct)
Correct. Less than 50 percent myometrial invasion is stage IA.
C. Stage II
Stage II needs cervical stromal invasion.
D. Stage IIIA
Stage IIIA needs serosal or adnexal involvement.
