Pathology · Inflammation and repair: Hageman factor
Sample itemWhich plasma protein, activated on contact with exposed collagen or basement membrane, initiates the kinin, clotting and fibrinolytic systems at a site of injury?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
A. Hageman factor (factor XII)
Hageman factor (factor XII) is activated by contact with negatively charged surfaces such as exposed collagen. It starts the kinin system, which produces bradykinin, and the clotting and fibrinolytic cascades, linking inflammation with coagulation. Kallikrein generated in the process also amplifies the cascade.
Option by option
A. Hageman factor (factor XII) (correct)
Correct. Factor XII is the contact-activated link between these plasma systems.
B. Factor VIII
Factor VIII is a cofactor of the intrinsic tenase complex and does not initiate contact activation.
C. Protein C
Protein C is an anticoagulant that inactivates factors Va and VIIIa.
D. Factor XIII
Factor XIII cross-links fibrin and acts at the end of coagulation.
