OBG · Gynaecological endocrinology: Kallmann syndrome
Sample itemA tall 17-year-old girl with primary amenorrhoea, absent breast development and anosmia has low FSH, low LH and low oestradiol. What is the cause?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
A. Isolated GnRH deficiency (Kallmann syndrome)
Kallmann syndrome results from failure of GnRH neurone migration, causing hypogonadotropic hypogonadism with anosmia or hyposmia. Treatment is pulsatile GnRH or gonadotrophins for fertility, and oestrogen for puberty.
Option by option
A. Isolated GnRH deficiency (Kallmann syndrome) (correct)
Correct. Anosmia with low gonadotrophins indicates Kallmann syndrome.
B. Polycystic ovary syndrome
PCOS shows normal puberty and high LH.
C. Gonadal dysgenesis
Gonadal dysgenesis gives high gonadotrophins.
D. Mullerian agenesis
Mullerian agenesis has normal breast development.
