OBG · Gynaecological oncology: radical hysterectomy
Sample itemDuring hysterectomy, the ureter is most vulnerable to injury at which site?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
D. Where it passes beneath the uterine artery
The ureter passes under the uterine artery about 1.5 to 2 cm lateral to the cervix, described as water flowing under the bridge. It is also at risk at the infundibulopelvic ligament and in the ureteric tunnel. Cystoscopy can check ureteric patency.
Option by option
A. At the bladder trigone only
Trigone injury is unusual.
B. Near the renal pelvis
The renal pelvis is far from the operative field.
C. At the pelvic brim near the sacroiliac joint
The pelvic brim is a crossing but a less frequent site of injury.
D. Where it passes beneath the uterine artery (correct)
Correct. This is the classic site of injury.
