OBG · Gynaecological oncology: cervical cancer staging
Sample itemA woman has a cervical cancer extending to the upper two-thirds of the vagina and into the parametrium without reaching the pelvic wall (FIGO IIB). What is the treatment of choice?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
A. Concurrent chemoradiation with cisplatin
Locally advanced disease (IB3 to IVA) is treated with concurrent cisplatin-based chemoradiation, including external beam radiotherapy and brachytherapy. Surgery is reserved for early disease (IA to IB2/IIA1). Stage IIB involves the parametrium, so surgery is not suitable.
Option by option
A. Concurrent chemoradiation with cisplatin (correct)
Correct. Chemoradiation is standard for stage IIB.
B. Palliative care only
Curative therapy is feasible.
C. Wertheim's hysterectomy
Wertheim hysterectomy is for stage IB to IIA1.
D. Simple hysterectomy
Simple hysterectomy is inadequate for stage II.
