Pediatrics · Nephrology: UTI imaging
Sample itemAfter a first febrile UTI in a 6-month-old infant, which investigation identifies vesicoureteric reflux?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
B. Micturating cystourethrogram
MCU demonstrates reflux and posterior urethral valves, and is done once the infection has settled. Ultrasound of the kidney and bladder is the first imaging study. DMSA scan detects renal scarring 4 to 6 months after infection.
Option by option
A. Intravenous urogram
IV urography is not used for this indication.
B. Micturating cystourethrogram (correct)
Correct. MCU grades reflux.
C. Plain abdominal X-ray
Plain films do not show reflux.
D. Serum creatinine
Creatinine does not locate reflux.
