Radiology · Neuroimaging: Sturge-Weber syndrome
Sample itemA child with a port-wine stain on the face and seizures has a skull radiograph showing gyriform tram-track calcification in the parieto-occipital region. What is the diagnosis?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
C. Sturge-Weber syndrome
Sturge-Weber syndrome features a leptomeningeal venous angioma with underlying cortical calcification, which gives the tram-track pattern, with a facial naevus in the trigeminal territory. Glaucoma and hemiparesis can occur. MRI with contrast shows leptomeningeal enhancement.
Option by option
A. Neurofibromatosis
Neurofibromatosis shows optic gliomas and neurofibromas.
B. Ataxia telangiectasia
Ataxia telangiectasia shows cerebellar atrophy and telangiectasias.
C. Sturge-Weber syndrome (correct)
