General Surgery · Urology: renal tuberculosis
Sample itemA young man has frequency, sterile pyuria with acidic urine and a contracted bladder. Cultures for routine bacteria are negative. What is the diagnosis?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
C. Genitourinary tuberculosis
Sterile pyuria with an acid urine and haematuria suggests genitourinary TB, which spreads haematogenously to the kidney. Three early morning urine samples are cultured for acid-fast bacilli, and imaging may show calyceal erosion or the thimble bladder. Treatment is a standard antitubercular regimen.
Option by option
A. Bacterial cystitis
Bacterial cystitis grows organisms on routine culture.
B. Interstitial cystitis only
Interstitial cystitis does not cause pyuria.
C. Genitourinary tuberculosis (correct)
Correct. Sterile pyuria with a contracted bladder suggests GU TB.
D. Urolithiasis
Stones do not explain sterile pyuria alone.
