Radiology · Renal imaging: ureteric jets on Doppler
Sample itemA patient with acute loin pain has an ultrasound showing mild right hydronephrosis. Colour Doppler of the bladder shows ureteric jets on the left but none on the right. What does this suggest?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
D. Complete or high-grade obstruction of the right ureter
A ureteric jet is the intermittent flow of urine into the bladder, seen on colour Doppler. Absence on one side with ipsilateral hydronephrosis suggests complete obstruction, whereas a weak continuous jet suggests partial obstruction. It is a useful adjunct when ultrasound is the first test.
Option by option
A. Bladder outlet obstruction
Bladder outlet obstruction would affect both sides.
B. Normal findings
The jets are expected on both sides in normal individuals.
C. Renal vein thrombosis
Renal vein thrombosis is not assessed by ureteric jets.
D. Complete or high-grade obstruction of the right ureter (correct)
Correct. An absent jet with hydronephrosis suggests complete obstruction.
