Psychiatry · Organic mental disorders: conduction aphasia
Sample itemA 58-year-old man after a left parietal infarct speaks fluently with phonemic errors, understands commands well, but cannot repeat the sentence 'No ifs, ands or buts'. Which aphasia is this?
Original practice item, not a recalled exam question. Nothing here is recorded.
The answer and the reasoning
B. Conduction aphasia
In conduction aphasia, the lesion lies in the arcuate fasciculus or supramarginal region, disconnecting the Wernicke and Broca areas. Speech is fluent and comprehension good, but repetition is disproportionately impaired, with phonemic paraphasias.
Option by option
A. Wernicke aphasia
Wernicke aphasia has fluent speech but impaired comprehension.
B. Conduction aphasia (correct)
Correct. Fluent speech with good comprehension but poor repetition is the hallmark.
C. Broca aphasia
Broca aphasia is non-fluent, effortful speech with relatively preserved comprehension.
D. Global aphasia
Global aphasia impairs both production and comprehension severely.
